First, your Instagram account needs to be a professional account (Business or Creator). Switching is free: in the Instagram app, open Settings, then Account type and tools, then Switch to professional account. Then open Settings in your Carti dashboard, find "Where should Carti answer shoppers?", click Connect on the Instagram row, and log in with Instagram. That is the whole setup, Carti subscribes to your messages automatically. To check it works, send your account a DM from another account. Carti answers within a few seconds. If it does not, confirm the account type, then in the Instagram app open Settings and activity, then Messages and story replies, then Connected tools, and turn on Allow access to messages (that menu can take a few minutes to appear).
